For Health Professionals

There are big differences between the home health care providers you offer your patients.

Home Care Standards Bureau A+ Rating

A+ Rating for Fort Collins Home Health Care Providers from Home Care Standards Bureau

The HCSB evaluates home health care providers based on twelve key standards to attain their A+ rating.
Some of those standards include:

  • The agency must agree to rigorous scheduled and unannounced on-site audits.
  • A minimum of 90% of employees must be W2 employees of the agency.
  • All caregivers employed by the agency must have passed a multi-tiered background check, including, but not limited to, a 50 state multi-jurisdictional, criminal, sex offender registry and the current state driving record.

“Vitality Home Healthcare is the only home healthcare provider in Northern Colorado to receive an A+ rating.”

Re-admission Rates

Re-admission rates to the hospital can be one of the most telling statistics in determining how well a home healthcare provider is offering the standard of care you expect for your patients.

Re-admission rates can cost you money. Medicare and Medicaid can penalize a hospital or clinic if the patient is readmitted within 30 days of discharge.

Poor re-admission rates from home health care providers may impact how your clients judge your quality of care.

Fort Collins home health care providers

Are your patients getting the quality home healthcare you think they’re getting?

In our Medical Care Division, we don’t believe in “drive-by-nursing,” where home health care providers offer incentives to their staff to make as many calls as they can, as quickly as they can. Our nurses spend the necessary time it takes to know your patients. They routinely go above and beyond what would be considered “adequate care.” They give superior care.

Why is Vitality™ different from all the rest?

Within our Personal Care Division, the vast majority of our care givers are Certified Nurse Assistants. With every call they make, however routine it might be, they always carry their “black bag.” Numerous times they have helped spot and prevent serious potential medical problems before they started.

We believe in keeping you informed. From the moment we begin with your patients until we discharge them, we are in regular communication with you. We don’t like unpleasant surprises. We believe you don’t, either.

Because we at Vitality™ are committed to quality care for your patients, you can enjoy peace of mind in knowing your patients—and the reputation of your clinic or practice—are in good hands.
